feat(report): add report schema identity

Summary

• add stable schema and schema_version fields to report.json
• include the new fields in report contract extraction and golden JSON fixtures
• document the JSON artifact contract and changelog entry

Validation

• git diff --check
• cmake --build build
• ctest --test-dir build -C Debug --output-on-failure
• privacy/secret scan over changed files
